It is frustrating. Treating with insulin is not an exact science and blood sugar is a constantly moving target.
Some things that effect blood sugar that you may not be aware of are menstrual cycle hormones (most women require more insulin when they are premenstrual), stress - even a little extra stress can make you resistant to insulin, illness - even a little sore throat, and acetaminophen - this can give you a falsely high reading. Exercise can effect readings for 24 hours.
All you can do is your best, and it sounds like you are doing that. Also, keep reviewing your readings with your care provider - sometimes someone outside can see something you can't.
