It is NOT possible. With few exceptions, that is why most Type 1 diabetics in poor countries where fast acting insulin is not available die of horrible complications in a fairly short period of time. In some countries like Japan, where Type 1 diabetes is considered a stigma and a "curse" by some families, the child/person is often given very little insulin and/or perhaps only long acting insulin. It is not uncommon in these cases for the child to live less than 5 years. Type 1 children in Africa often die soon after diagnosis due to lack of proper insulin to treat it (DKA needs fast acting insulin to bring the person out a the coma in time to save their life).

The problem is sustained high blood sugars, but also potentially severe lows from the unpredictability and long action of NPH. This is made worse if the person is not able to test their blood sugar regularil or eat regular meals.

So you can keep a person alive (but not well) on NPH and a strict low carb diet. But by no means will they have "good control" or a good quality of life. Or likely a long life.
